Answer

Design refreshes the design to any changes that was made to the template it was based on

nconvert converts the db to the specified template and changes the based on template so when the design task runs and if changes where made to the template the changes will be refreshed in the database. There are also switches in nconvert that will upgrade various items such asupgrade folders, add languages and imap support
